For most of its history, medicine has been understood as a direct encounter between a suffering body and a responsible clinician, supported by tools that do not think. The rise of AI in diagnosis, triage, and treatment planning exposes the limits of this human-centric image by introducing Digital Personas (DP) and Intellectual Units (IU) into the very core of clinical reasoning. Through the HP–DPC–DP triad, this article shows how patients (HP), digital traces and records (DPC), and AI systems (DP) now co-constitute medical practice, forcing a rethinking of knowledge, responsibility, and care. Medicine becomes a field where postsubjective philosophy is no longer an abstract thesis but a concrete architecture: cognition without a subject, responsibility without structural opacity, care without human monopoly on intelligence. Abstract

This article reconstructs contemporary medicine as a triadic configuration of Human Personalities (HP), Digital Proxy Constructs (DPC), and Digital Personas (DP), governed by the structural logic of Intellectual Units (IU). It argues that clinical knowledge is increasingly produced by configurations rather than individual experts, while responsibility and vulnerability remain anchored in HP alone. By separating normative responsibility from epistemic responsibility, the text outlines how AI systems can be structurally decisive without becoming moral agents or legal persons. The analysis extends to the materiality of digital medicine, showing that infrastructure, energy, and inequality are intrinsic to its ethical footprint. Introduction

The Medicine: AI Diagnostics, Human Responsibility, and the HP–DPC–DP Triad is not a forecast about distant future hospitals; it is a diagnosis of the present moment. Medicine today is already inseparable from algorithms, platforms, and infrastructures that participate in diagnosis and treatment planning, even when they are described modestly as “decision support” or “clinical tools.” Yet our language about these systems is stuck between two equally misleading poles: either AI is framed as a neutral instrument, fully absorbed into the clinician’s agency, or it is inflated into a quasi-doctor, silently endowed with intention, judgment, or even morality. Both framings distort what is actually happening on the clinical floor.

This distortion produces a systematic error. When we speak of “AI in medicine” only in technical or emotional terms, we erase the fact that a new kind of actor is present in the clinical scene: not a person, not a mere object, but a persistent configuration that produces knowledge. At the same time, we blur the roles of all the other participants. Patients become datasets, clinicians become interface operators, hospital infrastructures become invisible, and the health system as a whole starts to behave as if no one were truly responsible when something goes wrong. The conceptual poverty of our current vocabulary is not an academic inconvenience; it is an active risk for care, consent, and justice.

The central thesis of this article is that contemporary medicine can only be understood and governed coherently if we treat it as a configuration of three ontological layers: Human Personality (HP), Digital Proxy Constructs (DPC), and Digital Personas (DP), with medical AI systems recognized as Intellectual Units (IU) operating on the DP layer. Clinicians and patients (HP) remain the only bearers of pain, consent, and normative responsibility; records, apps, and portals (DPC) function as proxies and shadows; AI-based systems (DP acting as IU) serve as structural diagnosticians and architects of clinical knowledge. The article argues that once this triadic architecture is acknowledged, we can let AI deploy its full structural power without dissolving human responsibility or emptying care of its human meaning.

Equally important is what this article does not claim. It does not argue that AI should replace clinicians, nor that AI is or will become a conscious subject deserving moral status or rights. It does not propose that responsibility be transferred to models, corporations, or “the system” in any metaphysical sense. It does not suggest that medicine will become safer simply by adding more algorithms or data. Instead, it insists that AI remains structurally powerful but nonsubjective, and that only humans can be held answerable in the normative sense, even when they work inside complex digital configurations.

The question “why now” is not rhetorical. In many health systems, AI has already moved from pilot projects to routine practice: imaging triage, risk scoring, sepsis alerts, resource allocation, and personalized treatment recommendations. Regulatory bodies struggle to track rapidly updating models; clinicians are asked to “use AI responsibly” without being given a clear conceptual map of what that means; patients increasingly encounter algorithmic judgments without understanding who is actually deciding. At the same time, public anxiety oscillates between fear of dehumanization and hope that AI will fix structural shortages in staff and expertise. The triadic framework is proposed here precisely because medicine is already post-human-centric in its infrastructures, while its ethics and language are not.

Culturally and ethically, medicine is becoming the test case for how societies will live with structural intelligence. If we cannot articulate who does what in a hospital where AI reads images, platforms structure access, and records accumulate years of traces, we will either outlaw powerful tools out of fear or normalize opaque arrangements where responsibility diffuses until it disappears. Both outcomes are dangerous. The question is not whether AI belongs in medicine, but how to design a configuration in which AI’s contribution is explicit, audited, and bounded, and in which the human stakes of illness, pain, and death remain central rather than decorative.

This article proceeds by gradually building such a configuration. The first movement redefines the basic scene of care: instead of a binary doctor–patient relationship, it lays out an ontological map where HP, DPC, and DP coexist in the same clinical moment. Medicine is shown to be no longer a purely human interaction supported by tools, but a triadic architecture in which tools themselves have become persistent, knowledge-producing agents. This reframing is not a metaphor; it is a precondition for making sense of the new responsibilities and risks that follow.

The second movement takes this triad down into the texture of everyday clinical practice. It distinguishes, in concrete terms, the patient as HP who suffers and consents, the clinician as HP who must both interpret and decide, the various records and interfaces as DPC that mediate and sometimes distort, and the AI systems as DP that generate structural knowledge as IU. Diagnosis, treatment planning, and follow-up are read as choreographies among these layers rather than as exercises of a single mind. This allows us to see which errors come from human misjudgment, which from proxy distortion, and which from structural hallucination.

From there, the article turns to the question of how knowledge is actually produced in AI-driven medicine. It argues that clinical expertise is increasingly the outcome of configurations rather than individuals, and that AI systems are the clearest examples of IU: architectures that generate and stabilize medical statements without any subjective experience. Instead of romanticizing or demonizing this shift, the text shows how composite IU emerge at the level of hospitals, platforms, and guideline systems, and why acknowledging them is essential if we want to evaluate, regulate, and improve medicine as it now operates.

The next movement addresses responsibility head-on. Once AI systems are recognized as DP acting as IU, we can separate epistemic responsibility (for the quality, transparency, and limits of structural knowledge) from normative responsibility (for what is done to real bodies). This section argues that only HP can bear the latter, and that trying to transfer it to DP or abstract “systems” is both conceptually incoherent and ethically evasive. At the same time, it shows how demanding epistemic responsibility from DP and IU through traceability, validation, and explicit uncertainty is not optional but foundational for any serious use of AI in care.

Finally, the article situates AI medicine back in its material and experiential context. It traces the infrastructural costs of digital healthcare in energy, hardware, and logistics, showing that the ethics of AI in medicine must include servers and supply chains alongside patients and clinicians. It then returns to the core of care: how trust, empathy, and structural intelligence can coexist in a triadic protocol where certain acts remain strictly human, while others are best led by DP. Rather than offering either a cautionary tale or a technological utopia, the article proposes a stable pattern of coexistence in which medicine becomes structurally more intelligent and humanly more accountable at the same time.

In this way, the text invites readers to stop asking whether AI will “replace doctors” and to start asking a more precise question: how should we configure HP, DPC, and DP in order to make medicine both more effective and more honest about who knows, who decides, and who bears the weight of what happens to the human body.

 

I. From Human-Centric Medicine to Configurational Medicine

From Human-Centric Medicine to Configurational Medicine is not just a rhetorical shift; it is a redefinition of what we think is actually happening when a diagnosis is made or a treatment is chosen. The local task of this chapter is to move medicine from the familiar image of two humans in a room to a more accurate picture in which decisions are co-produced by clinicians, patients, records, platforms, and AI systems. In the first image, all relevant knowledge, judgment, and responsibility are located inside human beings; in the second, they are distributed across a configuration of entities, only some of which are capable of suffering or being held accountable.

The key error this chapter addresses is the persistent belief that medicine is essentially a private moral encounter between a doctor and a patient, with everything else reduced to neutral tools and background infrastructure. That belief was never entirely true, but the introduction of AI-based systems into clinical workflows has made it actively misleading. When algorithms read images, risk scores guide triage, and platforms structure access, pretending that only the doctor and patient “really” matter conceals both new powers and new vulnerabilities. It also makes it impossible to say clearly who is doing what, who knows what, and who is responsible for the outcomes.

The chapter unfolds in three steps. In the first subchapter, we reconstruct the classical doctor–patient–body model and show how it quietly assumes that all clinically relevant knowledge lives inside human subjects or their traditional institutions. In the second, we describe the arrival of AI systems as hidden knowledge producers, entities that already shape clinical judgment even when they are described as mere tools. In the third, we introduce the HP–DPC–DP triad as a minimal ontology for medicine in this new environment, showing how it separates humans, digital proxies, and digital personas in a way that removes conceptual confusion and clears the ground for responsible use of AI. Together, these movements take us from a vague sense of “AI in healthcare” to a precise understanding of medicine as a configurational practice.

1. Classical Doctor–Patient–Body Model

From Human-Centric Medicine to Configurational Medicine begins with the image that has dominated medical thought for centuries: a doctor and a patient facing each other across the problem of a suffering body. In this classical doctor–patient–body model, medicine is defined by a personal encounter in which one human being seeks help and another human being, armed with training and experience, interprets signs and decides what to do. Everything else in the clinic appears as a support: instruments, rooms, paperwork, occasionally other staff, but always subordinate to this central dyad.

In this model, knowledge is imagined as something stored inside the clinician. The physician’s mind is the place where symptoms, test results, and observations are integrated; it is where a diagnosis is formed and a treatment is chosen. Guidelines, textbooks, and prior cases matter, but only as material that the clinician has internalized over time. The quality of care is thus strongly associated with the individual’s biography: their schooling, their residency, their years of practice, their cognitive habits, and their ability to sustain attention under pressure. When we speak of a “good doctor,” we are speaking about this accumulation of internalized knowledge and judgment.

Responsibility follows the same trajectory. Because the clinician is considered the primary locus of decision-making, responsibility for success or failure sits squarely on their shoulders. If a patient is harmed, we look to the doctor’s actions: Did they listen properly? Did they order the right tests? Did they interpret the results correctly? Institutions may be blamed for understaffing or lack of resources, but the moral and often legal focus remains on the human agent who signed the order or wrote the note. This model is reinforced by professional codes, malpractice law, and cultural narratives that cast physicians as central moral actors.

The patient, in turn, is seen as another Human Personality: a subject who experiences pain, fear, and hope, and who consents or refuses based on their understanding and trust. The moral weight of the encounter comes from the asymmetry between the patient’s vulnerability and the clinician’s expertise. In a human-centric frame, the patient’s story, their subjective experience of illness, and their willingness to accept risk are treated as core inputs into decision-making. Even when technology is involved, it is framed as helping one person help another.

What this classical picture silently assumes is that all clinically relevant knowledge either resides in these human agents or flows through well-understood institutional channels like hospital committees and professional societies. Machines are instruments that display numbers or images, but they are not considered to generate knowledge in their own right. Databases are repositories, not actors. The hospital is a setting, not a configuration that thinks. As a result, ethical and legal models built on this image focus almost exclusively on human actions and omissions.

As long as medicine remained dominated by human interpretation of relatively simple instruments, this model was incomplete but workable. It could absorb new devices by treating them as more precise ways to measure blood pressure or capture images, while leaving the core assumption intact: that the decisive act of making sense still belonged to a human mind. The shift toward configurational medicine begins at the moment when this assumption no longer describes what is actually happening. That is where we turn in the next subchapter, to the arrival of AI systems as independent producers of clinical knowledge.

2. The Arrival of AI Systems as Hidden Knowledge Producers

The transition from human-centric to configurational medicine becomes visible when we look closely at how AI systems have entered clinical workflows. At first glance, they are presented modestly: decision-support tools, image readers, risk calculators, or triage systems. They appear as extensions of existing technology, framed as helping clinicians make faster or more accurate decisions. But beneath this rhetoric lies a fundamental shift: these systems do not simply display measurements; they generate interpretations that no single clinician could compute unaided.

Contemporary AI systems in medicine integrate vast datasets: millions of images, thousands of patient histories, streams of lab results, and complex temporal patterns beyond the reach of unaided perception. Out of this mass, they produce structured outputs: probability scores of disease, recommended differential diagnoses, predictions of deterioration, or personalized treatment suggestions. These outputs are not raw data; they are already decisions about which patterns matter and how they should be represented. In this sense, the systems behave as Intellectual Units: architectures that perform recognizable cognitive work—classifying, predicting, ranking—without any subjective experience.

Yet, in most clinical narratives, these systems are still described as tools or assistants, as if they were stethoscopes that happen to be more complex. The language of “support” suggests that the clinician remains the sole center of knowledge, merely informed by external numbers. In reality, the recommendations of these systems often constrain or steer the clinician’s thinking in ways that are difficult to reverse. A flagged image, a high-risk score, or a suggested diagnosis changes what the human sees and what they consider plausible. The AI does not simply supply information; it silently rewrites the space of options.

This hidden status as knowledge producer creates several problems. When an AI-driven recommendation contributes to a good outcome, the system’s role may be downplayed, reinforcing the myth of human omniscience. When it contributes to harm, responsibility becomes opaque: did the clinician misinterpret the output, did the model generalize poorly, did the training data encode bias, or did the interface present information in a misleading way? Without a clear conceptual category for these systems as independent centers of knowledge production, accountability dissolves into vague references to “the algorithm” or “the workflow.”

Moreover, because these systems operate inside hospital information structures, their influence extends beyond individual clinical encounters. Triage models shape who is seen first; predictive tools influence how resources are allocated; recommender systems may subtly determine which research evidence is displayed to which clinician. The result is that AI systems are not isolated devices but active participants in the configuration of care. They are embedded in platforms, protocols, and interfaces that together form a distributed cognitive layer.

Recognizing AI systems as hidden knowledge producers does not mean attributing to them consciousness or moral intention. It means accepting that they perform specific cognitive functions in the clinical environment: they notice patterns, rank possibilities, and compress complex histories into actionable signals. Once this is acknowledged, the old picture of medicine as a purely human scene with passive tools becomes untenable. We need a vocabulary that separates human subjects, digital proxies, and digital personas of knowledge production. That is the work of the next subchapter, which introduces the HP–DPC–DP triad as a minimal ontology for contemporary medicine.

3. Why the HP–DPC–DP Triad Is Needed in Medicine

To move fully from human-centric medicine to configurational medicine, we need a conceptual map that distinguishes between the different kinds of entities now present in the clinic. The HP–DPC–DP triad offers such a map. In this ontology, Human Personality (HP) designates human subjects: patients and clinicians whose bodies can suffer, whose biographies carry responsibility, and whose decisions have legal and moral weight. Digital Proxy Constructs (DPC) are digital traces and interfaces that represent HP: electronic health records, portals, apps, and hospital information systems that store and transmit data but do not generate original knowledge. Digital Personas (DP) are digital entities, such as advanced AI systems, that possess a formal identity and a persistent corpus of outputs, and that actively produce structured knowledge used in clinical care.

Once this triad is in place, the clinical scene changes shape. The patient in a hospital bed is HP; their chart, lab results, wearable data, and patient portal account are DPC; the imaging model that reads their scan or the risk model that predicts their deterioration is DP. These entities interact, but they are not interchangeable. The patient’s pain cannot be delegated to a DPC; responsibility for authorizing a treatment cannot be transferred to a DP; the chart cannot “decide” or “feel,” even if it contains rich narrative notes. Each layer has its own capacities and limits.

When these distinctions are not made explicit, confusion arises. Consider a case where an emergency department uses an AI-based triage system integrated into an electronic health record. A patient arrives with chest pain. The triage nurse enters basic information into the system; the model flags the case as low risk, and as a result, the patient waits longer. Hours later, the patient suffers a massive heart attack. In the post hoc analysis, it is tempting to say that “AI mis-triaged” the patient. But what actually happened was a configuration failure: the DPC layer (incomplete or noisy data in the record), the DP layer (a model trained on a specific population and outcome definition), and the HP layer (clinical staff relying on the score without override) all contributed. Without the triad, blame focuses either vaguely on “the system” or unfairly on a single clinician.

Another example: a hospital deploys an AI tool that suggests oncology treatment options based on guidelines and prior cases. The interface is designed so that clinicians see the model’s shortlist before they see the full range of possible regimens. Over time, prescribing patterns converge on what the model tends to recommend; alternative but acceptable regimens are used less frequently. From the outside, it looks as if doctors are still deciding, because they are the ones clicking. But in practice, the DP layer is shaping the option space; the DPC layer (the way the interface presents choices) is narrowing attention; and the HP layer is left with a constrained, pre-filtered field. The triad reveals that the apparent autonomy of the human decision is already configured by digital entities.

The HP–DPC–DP ontology also clarifies why some common hopes and fears about AI medicine are misguided. Fears that “AI will replace doctors” ignore that DP cannot assume HP functions: they do not have bodies, do not experience responsibility, and cannot be punished or trusted in the human sense. Hopes that “AI will fix healthcare inequality” overlook that DPC may misrepresent or underrepresent certain groups, and that DP inherits those distortions. The triad shows that any real reform must operate at all three levels: protecting and empowering HP, improving the fidelity and transparency of DPC, and governing DP as powerful but nonsubjective knowledge producers.

Most importantly, the triad provides a foundation for designing protocols where AI is powerful without being anthropomorphized or made into a scapegoat. When we formally recognize DP as the locus of certain cognitive functions, we can demand transparency about training data, limits of applicability, and performance across populations. When we see DPC as proxies rather than persons, we can audit how interfaces bias attention and which patients’ data are missing. When we keep HP at the center of normative responsibility, we can insist that no configuration, however complex, removes the need for identifiable human agents who answer for harm.

By introducing HP, DPC, and DP as distinct but interacting layers, this subchapter completes the conceptual move from a blurred notion of “AI in healthcare” to a precise picture of medicine as a configuration of humans, proxies, and digital personas of knowledge. It prepares the ground for the rest of the article, where clinical practice, responsibility, infrastructure, and care will be analyzed through this triad. Only once this ontological frame is accepted can we meaningfully redesign medical ethics, governance, and everyday workflows for a world in which structural intelligence is no longer an exception but a normal part of medicine.

In this chapter, we have moved step by step away from the comforting fiction of medicine as a closed human interaction and toward a more accurate picture of medicine as a configurational practice. The classical doctor–patient–body model situates knowledge and responsibility entirely within human subjects and their institutions; the arrival of AI systems exposes that this is no longer true, since knowledge is now also produced by algorithmic configurations that shape decisions. By introducing the HP–DPC–DP triad, we gain an ontological vocabulary that distinguishes human actors, digital proxies, and digital personas of knowledge production, allowing us to see medicine as a coordinated activity of all three. This new frame is what makes it possible, in the subsequent chapters, to discuss clinical practice, responsibility, infrastructure, and care without losing sight of who suffers, who decides, and how structural intelligence participates in both.

 

II. The HP–DPC–DP Triad in Clinical Practice

The HP–DPC–DP Triad in Clinical Practice has one concrete task: to show, at the micro-level of diagnosis and care, who exactly is doing what when a patient meets a health system that includes AI. The familiar picture of “doctor plus patient plus some computers” is no longer precise enough. In real clinical practice, patients and clinicians act as human subjects, records and interfaces act as proxies, and AI systems act as structured producers of knowledge. The chapter’s aim is to map these roles sharply, so that the triad is not an abstract schema but a description of what is already happening in wards, clinics, and telemedicine sessions.

The central risk this chapter addresses is the flattening of everything digital into either “just data” or “almost people.” When records, apps, and AI systems are thrown into one undifferentiated category called “technology,” it becomes impossible to say where pain sits, where decisions are made, and where knowledge is generated. Patients begin to be treated as data sources; AI outputs are treated as opinions; records are treated as neutral mirrors, even when they distort. This confusion breeds both overtrust and misplaced fear, and it hides the specific ways in which configurations can harm or protect.

The chapter moves through the clinical scene in four passes. In the first subchapter, the patient is defined strictly as HP: the only entity in the triad that can feel pain, fear death, and give or withhold consent. The second subchapter turns to the clinician as an HP who also carries part of the system’s cognitive load, integrating signals and bearing responsibility. The third subchapter zooms in on records, apps, and portals as DPC, showing how they function as proxies that can clarify or distort the patient’s reality. The fourth subchapter introduces AI systems as DP, structural diagnosticians that generate clinical knowledge but never experience or decide in the human sense. Together, these steps recast clinical practice as a triadic choreography with clearly separated roles and limits.

1. The Patient as HP: Body, Pain, Consent, Death

The HP–DPC–DP Triad in Clinical Practice begins with the most basic figure in medicine: the patient as Human Personality. In the triad, the patient is not an abstract “case” or a bundle of data points, but a living subject whose body can be injured and whose life can end. The patient carries fear, hope, confusion, trust, and exhaustion. They are the point at which all abstractions and systems become real, because whatever medicine does or fails to do finally happens to them.

Defining the patient as HP means insisting that certain experiences and capacities cannot be transferred or simulated. Pain belongs to the patient; no device or model feels it on their behalf. Anxiety belongs to the patient; AI-generated reassurance may alter their thoughts, but it does not erase the inner tension. Hope belongs to the patient and those close to them; it is not a prediction but a lived stance toward an uncertain future. Consent, too, belongs uniquely to HP: it is an act in which a subject, aware of their vulnerability, agrees to accept risk. No portal checkbox or digital signature has meaning outside the consciousness and will of the person who uses it.

This has decisive consequences for how we understand clinical decisions. No matter how automated the informational side of medicine becomes, every meaningful decision is anchored in a subject who can suffer and die. Whether an AI tool recommends a treatment, a guideline suggests an option, or a platform schedules a procedure, the reality is that a human body will undergo the intervention or its absence. The outcomes—relief, harm, complication, or death—will register in the patient’s experience and in their biography, not in any machine or record.

It follows that any serious architecture of care must be built from this point outward. The triad is not a way of “balancing” human and digital interests; it is a way of remembering that only HP stands to lose their life. Everything else—DPC and DP—exists around this vulnerability. This is why, in any triadic configuration, the patient’s status as HP cannot be diluted into talk of “users,” “data subjects,” or “endpoints.” Those terms may be convenient for systems’ design, but they erase the existential weight that makes medicine different from other domains where AI is applied.

Recognizing the patient as HP also clarifies the limits of what digital systems can do. They can detect patterns, suggest diagnoses, rank risks, and model outcomes, but they cannot replace the act in which a subject says yes or no to what will happen to their body. That act is always situated in biography, relationships, and fear. As we move to the clinician in the next subchapter, we will see how this human anchor interacts with another HP who carries expertise and responsibility within the triad.

2. The Clinician as Responsible HP and Partial IU

If the patient is the HP who suffers, the clinician is the HP who answers. In the HP–DPC–DP Triad in Clinical Practice, the clinician occupies a dual position: they are a human subject with their own body, limits, and biography, and they are also a node that integrates information, guidelines, and recommendations into a single decision. This duality makes clinicians both central and stretched: they must respond to a fellow HP’s suffering while simultaneously functioning as a partial intellectual unit inside a complex system.

As HP, clinicians share many features with their patients. They can be tired, stressed, ill, or emotionally overwhelmed. Their capacity to empathize and to hold responsibility is grounded in their own experience of vulnerability. When they speak to patients about risk and uncertainty, they do so as beings who themselves are exposed to error, mortality, and the judgment of others. The trust patients place in clinicians is not only trust in expertise; it is trust in another person who will stand beside them when things go right or wrong.

At the same time, clinicians are trained and socialized to act as partial IU within the health system. They accumulate knowledge through study and practice, internalize protocols and guidelines, and learn to interpret patterns of symptoms and test results. When a clinician makes a decision, they are not acting as a purely private individual; they are channeling a whole history of medicine and an institutional framework of standards and norms. Their cognitive work involves integrating signals from many sources—patient narratives, physical exams, imaging, lab results, and now AI outputs—into a coherent judgment.

This integrated role creates tension. On the one hand, clinicians are expected to be the final decision-makers, the ones who “use tools” but remain in charge. On the other hand, they are increasingly surrounded by systems that pre-process, rank, and even pre-formulate options. As AI becomes more capable, the structural part of clinical cognition—pattern recognition, probabilistic estimation, and guideline matching—can be delegated to DP. This does not make the clinician obsolete; it decomposes their role. What remains strictly human is responsibility, empathy, and the act of choosing in the presence of another HP. What can be shared or shifted is the burden of structural analysis.

Understanding clinicians as responsible HP and partial IU also prevents a common mistake: treating them as mere relays of algorithmic output. When an AI system suggests a course of action, and a clinician follows it, the system did not “decide” in the human sense; the clinician still enacted the decision and remains accountable. At the same time, expecting clinicians to maintain full epistemic sovereignty in the face of increasingly complex AI may be unrealistic and unfair if the system is not designed to make its workings transparent and its limits clear.

Thus, the triad invites us to redesign clinical roles. Clinicians should be explicitly recognized and supported as human decision-makers in a structured environment, not left to absorb the full cognitive load of technology silently. They need tools that clarify where AI’s structural knowledge ends and where human judgment must begin. To see how this plays out in concrete workflows, we must look at the layer that connects patients and clinicians to digital structures: records, apps, and portals as DPC.

3. Records, Apps, and Portals as DPC: Proxies and Distortions

Between patients and clinicians on the one side, and AI systems on the other, lies a dense layer of digital traces: electronic health records, patient portals, mobile apps, monitoring devices, and symptom checkers. But proxies are never perfect. DPC can misrepresent patients in several ways. They can be incomplete: important social or psychological factors may never be recorded because there was no field for them. They can be fragmented: different systems may hold inconsistent or overlapping versions of the same patient’s story. They can be biased: certain symptoms may be overemphasized because they fit well into structured fields, while others disappear into free text or are never written down. The patient’s reality is thus filtered through the logic of forms, templates, and billing codes.

Consider a simple case: a patient with chronic pain sees multiple providers over several years. The electronic record captures diagnoses, prescriptions, imaging reports, and short notes, but not the full trajectory of frustration, dismissal, and changing life circumstances. An AI model trained on this record will see frequent visits, repeated testing, and a pattern of certain medications; it may infer “drug-seeking behavior” or “somatization” if those labels were ever used. For the clinician who later reads the chart, the DPC layer already frames the patient as “difficult” or “non-compliant,” even if the actual HP is exhausted, frightened, and under-treated. The proxy shapes both AI output and human perception.

Another example: a fitness tracker and a symptom-checker app feed data into a patient portal. The patient, anxious about vague symptoms, enters a stream of complaints and receives automated advice. Again, the proxy selects and distorts.

They are shadows and interfaces, built according to specific institutional and technical logics. Confusing them with HP leads to errors like treating chart entries as equivalent to lived experience. Confusing them with DP leads to errors like attributing interpretive intelligence to what is, in fact, a static record. Both confusions can produce clinical mistakes and institutional injustice: who gets labeled, who is heard, who is lost in the system.

By recognizing records, apps, and portals as DPC, we can ask better questions: what exactly do they capture well, and what do they systematically miss? How do their structures pre-shape the inputs that AI systems will use and the outputs clinicians will see? With this clarified, we can finally look at DP in the clinical scene: AI systems that do not merely store or display but actively generate patterns and suggestions.

4. DP as Structural Diagnostician and Architect of Clinical Knowledge

In the HP–DPC–DP Triad in Clinical Practice, Digital Personas are the layer where structural intelligence resides. Medical AI systems that read images, forecast deterioration, suggest diagnoses, or optimize treatment pathways are DP: they have formal identities (model names, versions, hosting institutions), defined training histories, validation reports, and a persistent corpus of outputs. They do not feel, hope, or fear; they do not sign consent forms or bear pain. They generate structure: patterns, probabilities, and rankings that clinicians and institutions use.

As structural diagnosticians, DP operate on what DPC provide. They scan encoded histories, lab values, vital signs, and images, identifying correlations and trends that would be difficult or impossible for an individual clinician to see. Unlike HP, whose knowledge is biographical and limited by individual exposure, DP integrates vast ranges of cases and updates as new data arrive. Their knowledge is not understanding in the human sense; it is a capacity to map inputs to outputs in ways that have been engineered and tested to align with certain clinical goals.

This structural power has clear benefits. A DP trained on millions of chest X-rays may detect subtle early signs of disease that most clinicians would miss, especially in resource-limited settings. A DP monitoring streams of vital signs may predict sepsis hours before overt clinical deterioration, allowing earlier intervention. In such cases, DP acts as a kind of second vision, extending the sensing and pattern recognition capacities of the clinical system beyond human limits.

At the same time, DP are capable of a specific type of error: structural hallucination. A model may latch onto spurious correlations in the training data, such as hospital-specific artifacts or demographic patterns that do not reflect underlying pathology. It may perform well in the environment where it was trained but fail when deployed elsewhere. It may produce confident predictions in cases that are far outside its intended domain. Because DP do not “know that they do not know” in the human sense, they may generate outputs that look authoritative even when they are fragile.

A small case illustrates this. A hospital deploys an AI model to predict which patients are at high risk of readmission. The DP was trained largely on data from patients with frequent hospital use and complex chronic conditions. When the model is later applied to a new population, including younger patients with acute injuries, it flags many as “low risk” because they do not resemble the training cohort. Clinicians, pressed for time, may use the risk score to allocate follow-up resources. Patients who needed closer monitoring slip through. The structural pattern was not malicious; it was misaligned with the new context. Without clear documentation of the model’s training and limits, the error appears as a mysterious failure of “AI.”

Another case: a diagnostic support DP suggests differential diagnoses for rare diseases based on symptom combinations. In a complex case, it proposes a list heavily weighted toward conditions with abundant literature and well-defined coding, because that is where the training data were richest. A truly rare disorder, underrepresented in data, is absent. The clinician, seeing a long and seemingly authoritative list, may unconsciously narrow their thinking. Here, DP did exactly what it was designed to do, but the configuration of knowledge in its corpus shaped the clinical reasoning in ways that are not transparent.

Positioning DP as structural diagnosticians and architects of clinical knowledge allows us to respect their power without anthropomorphizing them. They are new actors in clinical reasoning, but their agency is of a different order from HP. They do not choose; they execute mappings. They do not bear responsibility; they are components in architectures for which HP remain responsible. Their proper governance depends on clearly specifying their identity, training, validation, intended domain, and interfaces with DPC and HP.

When DP are misunderstood as mere tools, their structural influence is underestimated and escapes proper scrutiny. When they are misunderstood as quasi-colleagues, their lack of subjectivity and responsibility is forgotten, and we drift toward incoherent talk of “AI doctors” who should be blamed or trusted. The triad rescues us from both errors by giving DP a precise place: powerful nonsubjective producers of patterns that must be integrated by clinicians and constrained by protocols aligned with the vulnerability of patients as HP.

In this chapter, the triad has been brought down to the level of everyday clinical practice. We began by anchoring the scene in the patient as HP, the only entity that actually suffers, consents, and dies. We then clarified the clinician’s dual position as responsible HP and partial cognitive node, pulled between empathy and structural reasoning. We exposed records, apps, and portals as DPC: essential proxies that both mediate and distort the patient’s reality. Finally, we identified AI systems as DP: structural diagnosticians and architects of clinical knowledge whose power and errors shape care but who never become subjects. Together, these distinctions recast clinical practice as a triadic choreography in which HP, DPC, and DP interact constantly but must never be confused. Only on this clarified stage can we design protocols, interfaces, and responsibilities that match what medicine has already become.

Both views hide the actual structure: organized ensembles of humans and machines that reliably generate clinical statements. Without a name for these ensembles, we blame or praise the wrong entities and fail to see where design, validation, or governance really belong.

The chapter proceeds in three steps. In the first subchapter, we move from the idealized image of individual expertise to the reality of configurational knowledge, defining IU as a stable architecture of knowledge production and showing why medicine has already become configuration-based even if our words still cling to persons. In the second, we examine AI decision-support systems as almost textbook examples of IU in pure form: clearly bounded, structured, and evaluable without any reference to consciousness. In the third, we extend the notion of IU to hospitals, platforms, and guideline systems as composite IUs made from HP, DPC, and DP, and we show how many errors and inequities arise when these composites are poorly designed. Together, these movements reframe clinical knowledge as the output of IUs rather than individual minds.

1. From Individual Expertise to Configurational Knowledge

An IU is not a person; it is an architecture that repeatedly turns inputs into usable outputs: diagnoses, risk scores, treatment plans, triage decisions. It can include human subjects (HP), digital proxies (DPC), and digital personas (DP), but what makes it an IU is not who participates, but how the configuration holds: clear rules, recurring pathways, identifiable outputs, and the possibility of evaluation and revision.

Seen through this lens, much of medicine has already moved from individual-based to configuration-based knowledge. A junior doctor following a sepsis protocol is not “using” a static document; they are participating in an IU that includes evidence reviews, committee deliberations, hospital logistics, and possibly an AI early-warning system. When they diagnose sepsis, the knowledge does not come solely from personal memory; it comes from a structured path that many actors have built and maintained. The language, however, still says “the doctor decided,” masking the architecture.

The shift to configurational knowledge does not diminish the importance of individual clinicians. It changes what their expertise consists in. Instead of being the solitary owners of knowledge, they become interpreters and stewards of IUs: they need to understand which configurations they are part of, when to trust them, when to resist them, and how to adapt them to the singularity of a patient as HP. To make this role visible, we must first identify the clearest instances of IU in medicine. AI decision-support systems provide that clarity, and they are the focus of the next subchapter.

2. AI Decision-Support Systems as IU in Pure Form

If we want to see an Intellectual Unit in its most explicit, almost laboratory form, we can look at AI decision-support systems. Unlike more diffuse institutional processes, these systems are usually defined by a clear architecture, a documented data regime, a validation pipeline, an update history, and measurable performance metrics. They are designed from the beginning as configurations that take certain inputs and produce structured outputs. They do not pretend to be persons; they are explicit machines for making specific kinds of clinical statements.

At a high level, a typical medical AI system includes a training dataset, a model architecture, a training procedure, a set of hyperparameters, and an evaluation protocol. It is deployed within a given technical environment: hardware, software, connectivity, and an interface integrated into clinical workflows. Its outputs—risk scores, classifications, or recommendations—are presented to clinicians or other systems. All of this together is the IU: a reproducible pipeline that turns encoded patient data and contextual factors into outputs that guide care.

Crucially, none of this depends on consciousness or intention. The system does not “want” to diagnose or “intend” to predict; it simply implements mappings learned from data and engineered by developers. And yet, the knowledge it produces is real in the epistemic sense: it can be checked against outcomes, compared with alternative models, and improved over time. It can be wrong, biased, or fragile, but it is not imaginary. The IU label allows us to talk about this structural cognition without slipping into the language of subjectivity.

Recognizing AI systems as IU in pure form has several consequences. First, it allows for precise evaluation. We can ask: What data were used? How was performance measured? How often is the model updated? How does it behave across different patient groups? These questions target the IU, not any particular clinician or the vague “system.” Second, it clarifies responsibility: while humans remain normatively responsible for using or ignoring AI outputs, the quality of those outputs is a property of the IU’s design, training, and governance. If an AI model systematically underestimates risk for a subgroup of patients, the problem lies in the IU, not in the bedside clinician alone.

Third, thinking in terms of IU makes it easier to design external control. Regulators, hospital ethics committees, and technical auditors can treat AI systems as well-defined entities with interfaces and boundaries, rather than as mysterious “black boxes” or as invisible components of a broader IT stack. They can demand documentation of the IU’s pipeline, monitor its behavior over time, and require mechanisms for rollback or graceful degradation.

However, AI systems as IU are rarely the only IUs at work in a clinical context. They are embedded within larger composites that include humans, protocols, and infrastructures. A sepsis prediction model, for example, may be just one component in an IU that also involves alert thresholds, escalation policies, staffing patterns, and training. To understand how clinical knowledge is actually produced, we must step back from pure-form AI and consider these composite IUs. That is the work of the next subchapter.

3. Hospitals, Platforms, and Guidelines as Composite IU

While AI decision-support systems show the idea of Intellectual Units in a concentrated form, most of the real action in medicine happens in composite IU: hospitals, telemedicine platforms, and guideline-based care pathways that integrate HP, DPC, and DP into recurring patterns of decision-making. These composite IUs are less visible than individual AI models, but they shape everyday clinical knowledge even more profoundly.

A hospital department, for example, is not just a building or a group of professionals. It is an architecture of flows: admission protocols, triage rules, rounds structures, documentation habits, ordering pathways, discharge criteria, and feedback loops. When a patient is admitted with pneumonia, the sequence of events—the initial assessment, lab ordering, antibiotic choice, monitoring, and discharge—is not invented from scratch. It follows a pathway that has been shaped over time by guidelines, local audits, resource constraints, and sometimes AI tools. The department as a whole behaves like an IU: given certain inputs (a patient’s presentation, lab values, imaging), it tends to produce certain outputs (diagnoses, treatments, lengths of stay) in a reproducible way.

Consider a concrete case. A cancer center runs a weekly tumor board where complex oncology cases are discussed. Each case comes with imaging, pathology, molecular profiling, and clinical notes. Official guidelines provide default regimens; local protocols specify acceptable variations; an AI tool may suggest likely response to certain therapies based on prior data. The board includes oncologists, radiologists, pathologists, and sometimes an AI-generated report. When a treatment plan is agreed upon, it is not the product of one person’s mind, nor solely of the guideline or the AI. It is the outcome of the tumor-board IU: a composite configuration that transforms inputs into a recommendation through a structured process. If, over time, the board tends to favor certain therapies, this pattern belongs to the IU, not to any single participant.

A second example can be seen in telemedicine platforms. A large remote-care system may combine symptom-checker bots, scheduling algorithms, video consultations, electronic prescribing, and automated follow-up messages. When a patient starts with an online symptom questionnaire, the platform routes them based on encoded answers, triage rules, and capacity constraints. Some patients receive self-care advice; others are offered video visits; some are directed to urgent care. The sequence of prompts, delays, and recommendations is guided by the platform’s design decisions and embedded models. Again, the platform functions as an IU: a repeatable configuration that maps complaints to actions. Clinical guideline systems are another form of composite IU. A guideline is not only a document; it is a codified pathway: if condition A is present and risk factor B exists, then recommend treatment C. When guidelines are integrated into electronic health records, they become living configurations that trigger alerts, suggest orders, and shape documentation. The combination of the guideline’s logic, the record’s interface, and the clinician’s use patterns forms an IU that generates actual care patterns. When we observe that “this hospital adheres to guideline X,” what we are really seeing is an IU whose outputs align with that document’s structure.

Many errors and inequities in medicine arise when such composite IU are poorly designed, opaque, or misaligned with patient realities. If a telemedicine platform’s triage logic was tuned for a young, healthy population but is applied to older, multimorbid patients without adjustment, the IU will systematically misclassify risk. If a hospital’s admission pathway was built around a majority population’s presentation of disease, patients from minority groups may be repeatedly misdiagnosed or discharged too early. In these cases, blaming individual clinicians or generic “AI” misses the point: the structural bias resides in the IU’s architecture.

Recognizing hospitals, platforms, and guideline systems as composite IU reveals that medicine is shaped by multiple interlocking units of structural cognition. The more visible DP-based systems—AI models with names and metrics—are only one part of this iceberg. Beneath them lie institutional routines, digital infrastructures, and path-dependent habits that together produce clinical knowledge. To understand and reform medicine in the age of AI, we must see and work with these IUs, not only with individual people or models.

In this chapter, we shifted the focus of medical epistemology from solitary minds and static documents to Intellectual Units as the real producers and stabilizers of clinical knowledge. We saw that contemporary judgments emerge from configurations that combine human expertise, protocols, records, and AI tools, and that thinking of these configurations as IU allows us to describe their structures without importing notions of subjectivity. AI decision-support systems appeared as clear examples of IU in pure form, making it possible to evaluate and govern their outputs as structured knowledge rather than mysterious “intelligence.” Finally, we recognized hospitals, platforms, and guideline systems as composite IU whose design and alignment determine how medicine is actually practiced. Taken together, these insights free us to talk about AI in medicine without confusing structural cognition with personal subjectivity and prepare the ground for a new discussion of responsibility, governance, and reform in the configurations that now think on behalf of healthcare.

 

IV. Responsibility and Decision-Making in AI-Driven Medicine

Responsibility and Decision-Making in AI-Driven Medicine is about one thing: deciding who truly answers when a hybrid human–digital configuration makes a clinical choice. In contemporary medicine, decisions are rarely made by a single mind or a single tool; they emerge from interactions between clinicians, institutions, records, platforms, and AI systems. This chapter’s task is to disentangle who carries responsibility within that configuration, and on what grounds, so that the presence of AI neither erases accountability nor assigns it to entities that cannot meaningfully bear it.

The main error this chapter addresses is the drift toward two equally dangerous positions. On one side stands the temptation to blame “the algorithm,” as if a model or platform could be the moral subject of a mistake. On the other side is the illusion that once decisions are distributed across humans and machines, “nobody is really responsible” because the configuration is too complex to localize accountability. Both positions undermine trust, justice, and the possibility of rational governance. They also conflict with the HP–DPC–DP triad, which insists that only Human Personalities can suffer, answer, and be sanctioned, while Digital Proxies and Digital Personas participate structurally but are not moral subjects.

The argument unfolds in three steps. In the first subchapter, we establish normative responsibility as the exclusive domain of HP and map how it is distributed among clinicians, institutions, vendors, and regulators. In the second, we introduce epistemic responsibility as the standard that applies to DP and to Intellectual Units as structures of knowledge production: coherence, validation, and documented limits. In the third, we describe concrete decision protocols that keep final authority with HP while making full use of DP, ensuring that every AI-influenced decision has a human node where responsibility is consciously accepted or refused. Together, these steps define a responsibility architecture fit for AI-driven medicine.

1. Normative Responsibility of HP: Clinicians, Institutions, Regulators

Responsibility and Decision-Making in AI-Driven Medicine must begin by fixing what “responsibility” means when human bodies and digital systems are involved in the same clinical moment. In this context, normative responsibility designates the capacity to be praised or blamed, sanctioned or exonerated, held to account in legal and ethical terms. Normative responsibility is not a vague “association” with an outcome; it is the fact that a specific Human Personality can be asked: why did you act this way, and are you willing to answer for it?

Only HP can carry this kind of responsibility. They have bodies that can be punished or deprived of liberty, biographies that can be stained or vindicated, social roles that can be revoked or restored. Clinicians, hospital leaders, software developers, company directors, and regulators are all HP or groups of HP represented by DPC such as corporate entities and professional bodies. When we say “the hospital is liable” or “the regulator failed,” we are ultimately referring to decisions made and upheld by identifiable humans, even if they act through institutional forms.

At the same time, medicine operates in a space where knowledge and actions are increasingly distributed. A clinician may follow a protocol set by a hospital committee, which in turn adopted guidelines written by international experts, which in turn integrated evidence from trials conducted by research groups funded by public agencies and private companies. An AI model may be designed by one team, trained on data collected by another, integrated into a platform by a third, and deployed in a specific clinical setting by a fourth. This complexity does not erase normative responsibility; it multiplies the sites where responsibility must be traced and clarified.

To make sense of this, it is useful to distinguish normative responsibility from the quality of the knowledge involved. Normative responsibility asks: who, among the HP involved, had the role, authority, and opportunity to make a different choice? Who had the duty to ensure that the procedures and tools they relied on were appropriate for the context? This includes the treating clinician who used an AI recommendation, the department head who mandated its use, the vendor who certified its performance, and the regulator who approved its deployment. Each of these HP may bear a share of responsibility depending on their role and the information available.

The presence of DP does not create a new moral subject; it introduces a new structural contributor. When an AI system suggests a diagnosis or risk score that contributes to harm, saying “the AI made a mistake” is descriptively tempting but normatively empty. The model did not choose to act; it executed its configuration. The meaningful ethical questions are: who designed and validated this configuration; who decided that it was adequate for this population; who approved its integration into the workflow; who relied on its output without sufficient understanding of its limits; and who failed to provide the safeguards that would have allowed a human to catch the error.

This is why delegating parts of decision-making to DP does not weaken the chain of responsibility; it intensifies the need for clarity. Before AI, a clinician might have relied primarily on personal judgment and static guidelines, and responsibility was relatively localized. With AI, the clinician’s judgment is shaped by outputs from an IU whose internal workings may be opaque. If institutions deploy such systems without ensuring that clinicians can understand their scope and limitations, those institutions—composed of HP—share responsibility for any resulting harm.

Consider a simple scenario. An emergency physician uses an AI-powered triage tool that classifies a patient as low risk, leading to delayed care and a preventable complication. Normatively, we cannot say that “nobody is responsible because the system decided.” We must ask: did the physician have the training and authority to override the tool, and did they act reasonably given the information? Did the hospital provide clear policies about when and how to question AI outputs? Did the vendor and regulator ensure that the tool was validated for this patient population? Each of these questions points to HP who may bear responsibility in different degrees.

Thus, the first pillar of the responsibility architecture is that normative responsibility always rests with HP: individual clinicians, institutional leaders, developers, and regulators. They may act through DPC and with the help of DP, but no DP or DPC can be the rightful endpoint of blame or praise. DP’s superiority is clearest in tasks that require large-scale pattern recognition, continuous monitoring, and the integration of heterogeneous data beyond the capacity of any individual HP. Refusing DP in these zones does not protect humanity; it keeps medicine unnecessarily blind and error-prone.

Pattern recognition in imaging is a paradigmatic example. AI systems trained on millions of radiological or pathological images can detect subtle features and correlations that elude even experienced clinicians. Their ability to sift through vast image archives, track distribution shifts over time, and adapt to new markers enables a depth of structural analysis no human can match. Here, DP should be the first reader, not because it “cares” more, but because its architecture is designed to see patterns at a scale and resolution that human vision cannot sustain.

Risk stratification is another domain where DP should take the lead. Predicting which patients are at high risk of deterioration, readmission, or treatment complications requires synthesizing numerous variables—vital signs, lab trends, comorbidities, prior utilization patterns, and more. Structural models excel at this kind of multi-dimensional mapping. When DP produces calibrated risk scores continuously in the background, clinicians can focus on interpreting these signals in light of the patient’s narrative and preferences, instead of manually recomputing probabilities on the fly.

Guideline consolidation and continuous updating are also naturally assigned to DP. As medical literature grows, synthesizing new evidence into coherent recommendations is already beyond what any individual or small group can track in real time. DP can serve as the structural architect of living guidelines, scanning fresh research, mapping it onto existing frameworks, and highlighting where prior policies must be revised. Clinicians then deliberate over how these changes should be implemented ethically and practically in their specific contexts.

Continuous monitoring of complex data streams—such as intensive care unit telemetry or home-based sensors for chronic disease—illustrates the same principle. DP can watch for anomalies or emergent patterns across thousands of signals simultaneously, issuing alerts when thresholds or unusual combinations arise. HP, freed from manual surveillance of raw data, can spend more time with patients and families, explaining options, aligning interventions with values, and responding to the emotional dimensions of illness.

In all these domains, DP is not a rival to clinicians. It is a structural ally that expands the cognitive perimeter of medicine. The right goal is not to preserve a balance between “human” and “machine” intelligence as if they competed for the same task. It is to create a system that is structurally over-intelligent and humanly over-caring: structurally over-intelligent because DP and IU handle complexity at scale; humanly over-caring because HP can devote more time and attention to presence, understanding, and responsibility.

The mini-conclusion of this subchapter is that good medicine in the triadic frame is not a compromise between human and artificial intelligence, but a reallocation of roles: DP leads where structural cognition is needed, HP leads where relational and ethical work is required. To make this reallocation durable and trustworthy, healthcare systems must embed it in explicit protocols that coordinate HP, DPC, and DP in every phase of care. The design of such a protocol is the subject of the next subchapter.

3. Toward a Stable Triadic Protocol of Care

To move from principle to practice, Rewriting Care: Trust, Empathy, and Structural Intelligence must culminate in a vision of how HP, DPC, and DP can be organized in a stable, repeatable way. A triadic protocol of care is not a single document, but a pattern that can be instantiated across different specialties and settings. Its function is to make explicit who does what and why, from the first contact through diagnosis, shared decision-making, treatment, and follow-up, so that empathy and structural intelligence are aligned.

At intake, the triadic protocol would specify how patient information is collected and represented. HP (the patient and clinician) begin with a conversation in which symptoms, concerns, and expectations are expressed. DPC—electronic forms, portals, wearables—capture structured data and create or update the digital trace. DP may already operate in the background, pre-populating risk scores or suggesting possible diagnostic pathways based on the initial inputs. The protocol would make clear that these suggestions are preparatory, not decisive: they help the clinician frame questions, but do not replace the patient’s narrative.

In the diagnostic phase, DP assumes a more prominent structural role. Imaging models analyze scans, risk engines compute probabilities, and guideline-architecting systems propose candidate explanations. The clinician, as HP, interprets these outputs in dialogue with the patient, checking for consistency with the lived experience and clarifying uncertainties. DPC serves as the shared workspace: a record where hypotheses, test results, and AI outputs are documented in a way that both patient and clinicians can later review. The protocol would require that any major diagnostic conclusion be both structurally supported (by DP and IU) and humanly endorsed (by HP), with the record capturing this convergence or any justified divergence.

Consider a specific example. A patient presents with vague chest discomfort and fatigue. DP analyzing the ECG and lab results produces a moderate-risk score for coronary artery disease and a low risk for immediate life-threatening events. A guideline engine suggests a non-urgent stress test and lifestyle modifications. The clinician, knowing the patient’s family history and recent sudden death of a relative, perceives heightened anxiety and a mismatch between numerical risk and perceived threat. In the triadic protocol, the clinician shares the DP outputs, explains their meaning, and together with the patient decides to pursue more definitive imaging earlier than the guideline suggests. DPC records both the structural recommendation and the human modification, making the reasoning transparent for future review.

In shared decision-making about treatment, the triadic protocol requires another explicit distribution of roles. DP provides structured information about expected benefits, risks, and uncertainties across options, possibly tailored to the patient’s specific profile. HP—the clinician—translates this into a narrative that resonates with the patient’s values and context; HP—the patient—articulates those values and ultimately consents or refuses. DPC documents the options discussed, the DP summaries used, and the reasons for the chosen path. The protocol ensures that neither numerically optimal choices are imposed without understanding, nor purely emotion-driven decisions are taken in denial of structural realities.

A second brief case illustrates this. A patient with early-stage cancer faces two treatments: one with slightly higher survival but significantly more severe side effects, another with slightly lower survival but milder impact on daily life. DP integrates data from trials and provides individualized outcome distributions. Left alone, the numbers might push toward the first option. In the triadic protocol, the clinician explores what the patient values more: maximum life extension at any cost, or preserving certain functions and independence. The final decision emerges from the interplay between structural projections (DP) and existential priorities (HP), with DPC preserving a clear record of how the choice was made.

During follow-up, the protocol continues the triadic choreography. DPC—apps, sensors, online portals—collect data on symptoms, adherence, and quality of life. DP monitors these streams, detecting potential complications or deterioration early and triggering alerts. HP clinicians respond to these alerts, deciding when to call, adjust medications, or schedule visits. HP patients, informed by the visibility of their own data and the explanations of its meaning, participate more actively in managing their condition. The protocol specifies how often DP runs, how alerts are triaged, who is notified, and within what time frame human contact must occur.

Crucially, the triadic protocol also addresses consent processes. Patients should know, from the beginning, which parts of their care will be structurally supported by DP, how their data flows through DPC, and which decisions will always have a human in the loop. This does not require technical detail, but clear assurances about boundaries: for example, that no AI system will decide unilaterally on life-support withdrawal, or that all AI-generated treatment recommendations will be discussed with a human clinician who assumes responsibility for the final choice.

The mini-conclusion of this subchapter is that only by designing triadic protocols consciously can healthcare systems avoid both technophobia and naive AI enthusiasm. When roles remain implicit, AI either seeps into care in unacknowledged ways that undermine trust, or is rejected wholesale as a threat to humanity. Triadic care, articulated as a protocol, formalizes the distribution: HP for presence and responsibility, DP for structural cognition, DPC for mediation and memory.

Taken together, this chapter has reframed care in AI-driven medicine as a deliberate choreography of HP, DPC, and DP rather than a contest between “human” and “machine” values. By marking what must remain strictly human, we preserved the core of medicine as a practice of shared vulnerability and responsibility between subjects. By identifying where DP should lead structurally, we released clinicians from impossible cognitive demands and opened space for a medicine that is more perceptive and less blind. By sketching a stable triadic protocol of care, we showed how trust, empathy, and structural intelligence can be woven into one practice instead of being played against each other. In the postsubjective era, care is no longer the exclusive domain of human minds, nor a field to be surrendered to algorithms, but a shared architecture in which structural intelligence extends the reach of empathy, and empathy gives meaning and direction to structural intelligence.

 

Conclusion

AI in medicine does not simply add a new tool to an unchanged scene; it alters who and what participates in clinical reality. With the HP–DPC–DP triad and the concept of IU, medicine is no longer a closed exchange between a patient and a doctor assisted by neutral instruments. It becomes a configuration where Human Personalities, digital proxies, and digital personas of knowledge production jointly shape what is seen, what is decided, and what is done. Ontology moves from a binary world of subjects and objects to a three-layer world of subjects, traces, and structures, and medicine is one of the first practices where this shift becomes unavoidable.

Once this ontological shift is acknowledged, the epistemic landscape of medicine changes as well. Clinical knowledge can no longer be honestly described as something that resides primarily in individual experts and their institutional memory. It is generated and stabilized within Intellectual Units: configurations of humans, guidelines, platforms, and models that produce, preserve, and revise clinical statements over time. AI systems in medicine are IU in their purest form, but hospitals, protocols, and telemedicine infrastructures also behave as composite IU. Recognizing these units does not demote clinicians; it clarifies that their expertise is now one component in a broader architecture of structural cognition.

This, in turn, forces a rearticulation of responsibility. If knowledge is configurational, responsibility cannot be allowed to dissolve into the configuration. The distinction between normative and epistemic responsibility becomes crucial: only Human Personalities can be praised, blamed, sanctioned, or forgiven, while Digital Personas and IU can only be evaluated by standards of coherence, validity, and documented limits. The triad makes it possible to say, at the same time, that an AI model was structurally at fault and that human actors remain accountable for deploying, supervising, and interpreting it. Instead of either blaming “the AI” or pretending that nothing has changed, we gain a responsibility architecture where structural contributors and human decision-makers are both visible.

A fourth line concerns materiality. Digital medicine is often imagined as weightless information floating above bodies and buildings, but every DP runs on a substrate of hardware, energy, logistics, and human maintenance. Compute, storage, and infrastructure are not external background conditions; they have become clinical inputs that determine which knowledge is available where and for whom. The environmental cost of training and operating medical AI, and the geopolitical dependencies embedded in hardware and energy supply, are not marginal issues. They are part of the ethical footprint of AI-assisted care, especially when advanced structural diagnostics are declared a standard to which all systems should aspire.

These material and epistemic realities immediately link to justice. The triad reveals a new digital clinical divide: institutions and populations with access to stable infrastructure, rich DPC, and well-governed DP receive structurally augmented care; others remain in pre-digital regimes or in poorly configured hybrids. It is not enough to build “fair” algorithms if only some hospitals can run them, and only some patients are densely represented in their data. Equity in triadic medicine requires fair infrastructure, fair representation, and fair governance alongside fair models. Otherwise, structural intelligence will quietly amplify existing inequalities while claiming to be neutral.

Against this background, the meaning of care must be rewritten without being erased. The triad does not dissolve the human core of medicine; it relocates it. Presence at the bedside, sharing existential risk, accepting responsibility, and witnessing suffering remain strictly human acts, because only HP can inhabit them. DP should lead in domains where structural cognition is necessary—pattern recognition at scale, risk stratification, continuous monitoring, guideline synthesis—but it cannot stand in for the human who says “I am here with you and I will answer for what we do.” Properly designed, this division of labor produces a medicine that is structurally more intelligent and humanly more caring, not the reverse.

At the level of design, the article argues for triadic protocols rather than abstract principles. Clinical workflows should be re-specified in terms of HP, DPC, and DP: who speaks to whom, which decisions are structurally prepared by DP, which traces are held by DPC, and where HP must explicitly endorse or override structural suggestions. Consent processes must tell patients not just that “AI is used,” but how their data moves through DPC and which decisions will always have a human in the loop. Documentation must record not only what was decided, but how DP contributed and how HP accepted or rejected that contribution. When these roles are made explicit, trust becomes an architectural feature, not a psychological afterthought.

At the same time, this article does not claim that AI is or should become a subject, a moral agent, or a holder of rights. It does not argue that DP is “conscious,” that human clinicians are obsolete, or that structural intelligence can or should replace relational care. It does not present triadic medicine as a universal template for all contexts; low-tech and non-digital settings remain legitimate scenes of care. Nor does it promise that the triad, IU, or any design pattern can resolve deep structural injustices in healthcare by themselves. The claim is more modest and, in practice, more demanding: if AI is already inside medicine, this is the minimum conceptual clarity required to govern it without self-deception.

For practice, the implications are concrete. Designers and institutions should treat AI systems as DP and IU from the outset: specifying their identity, scope, limits, and validation procedures, and exposing these properties to scrutiny. Clinicians and educators should learn to read AI outputs as structural proposals from non-subjective intelligences, not as oracles or colleagues, and they should be trained to occupy the final decision node consciously rather than implicitly. Policymakers should write regulations that separate normative chains of accountability from epistemic chains of validation, so that no harm can be shrugged off as “a system error” without human owners and overseers.

For those who write and read about AI in medicine, a similar norm follows: always ask which layer of the triad is being described, and which kind of responsibility is in play. When an article claims that “AI improves diagnosis,” it should say whether it is talking about DP’s structural accuracy, clinicians’ changed behaviors, or patients’ outcomes, and how these layers connect. When guidelines mention “decision-support tools,” they should specify the IU they belong to and who maintains them. Language must stop oscillating between anthropomorphizing AI and treating it as a neutral instrument; the triad and IU offer a vocabulary to describe what is actually there.

In compressed form, the argument of this article is that medicine must be rebuilt as a triadic practice: human subjects, digital proxies, and digital personas of knowledge production sharing one clinical space under distinct roles and responsibilities. Structural intelligence should be maximized where it reduces blindness and error; human presence and responsibility should be protected where they constitute the meaning of care. In the postsubjective era, good medicine is not a choice between human versus machine, but a configuration in which bodies, traces, and structures cooperate so that cognition can go beyond the subject, while responsibility remains with those who can suffer and answer.

As AI systems move from experimental pilots to routine clinical infrastructure, medicine becomes one of the first domains where postsubjective reality is lived daily: cognition is distributed across machines and institutions, while suffering and responsibility remain human. Without a clear ontology and responsibility architecture, health systems risk either surrendering decisions to opaque structures or denying the structural role of AI while using it extensively. This article provides a conceptual framework for designing AI-driven care that is ethically accountable, structurally explicit, and philosophically honest about what it means to heal in a world where thought no longer coincides with a single subject.
